Transition Portfolio Guidance

Transition Portfolio Guidance

The Indiana Department of Education's 2024 Transition Portfolio Guide provides a framework for creating student-centered transition portfolios for students with IEPs who are graduating with an Alternate Diploma. It covers key areas like student information, learning characteristics, academic and employability skills, with templates and examples to guide development. Transition portfolios help students showcase their strengths, skills, and goals for postsecondary success. Pre-ETS providers and school staff can collaborate by aligning IEP goals and transition needs with portfolio content, tracking progress, and tailoring work-based learning experiences to ensure consistency and better post-school outcomes. The portfolio also serves as a vital communication tool among students, families, educators, and employers that can be utilized beyond the time that a student exits school. Read the full guide here.
